I N S T R U C T I O N S 

Students enrolling in SBS's programmes in academic year 2019 are kindly requested to fill in and submit this form to register for SBS FOP 2019. For questions that do not apply to you, please key in "NA" and proceed to the next question. If you are not aware of any medical conditions or allergies, or are not aware of your blood type, please key in "Not Known" in the specified field. Do take note that there will be a programme fee of $45 per freshmen (inclusive of food and shirt costs), which will be collected on the first day of FOP.

*Note: Due to overwhelming sign ups, we will be closing SBS FOP registrations on 28th July 2019, 2359hrs. Please email BSCLUB-FOC@e.ntu.edu.sg if you require any assistance. 

For freshmen who do not stay in hall, please indicate if you would like to apply for short-term accomodations (STA) throughout the duration of the FOP. Do note that rates will be subjected for STA (on top of the $45 registration fee) and the application is non-refundable. Please refer to the rates as shown in the image below: 

For exmaple, if you are applying for a twin-sharing bedroom throughout the duration of FOP (3 nights), you will need to pay a total of $28.50 per pax for an aircon room (after subsidy), and $19.50 per pax for a non-aircon room (after subsidy).
