This form is applicable to students, regardless of nationality, who are polytechnic diploma holders or final year students from any of the five local polytechnics in Singapore; or who presents a local diploma from the Nanyang Academy of Fine Arts (NAFA), LASALLE College of the Arts or Building and Construction Academy (BCA); or who presents a Diploma in Sports Management & Exercise Science by the Singapore Sports School-Auckland University of Technology (SSS-AUT).
