Event Info Objective: The consensus-derived definition of multimorbidity is the presence of three or more chronic conditions from a list of 23 chronic. For a condition to be chronic, it should last for six months or more, be recurrent or persistent, impact patients across multiple domains and require long-term management.* The commonest triad of multimorbidity in primary care is hypertension, hyperlipidaemia and diabetes. The common complications resulting from these conditions are IHD, Stroke and CKD. The latter three conditions will have HeathierSG care protocols published in 2024. Hosted by Primary Care and Family Medicine: Assistant Professor Lee Eng Sing, Clincial Director, Primary Care and Family Medicine Research Programme Prof Lee is a local practising family physician in Singapore who has completed PhD in Family Medicine. In his training, focusing research that can be translated to patient-centred care is paramount. Being the head of Hougang Polyclinic for more than five years also allowed him to understand how the public primary care setting is run operationally and administratively. Eng Sing has also been working as a private GP before joining the polyclinic for 10 years and that experience helps him to understand the working environment of a solo GP. He is currently working closely with GPs for his role as a Care protocol sub-workgroup committee member in HealthierSG (under MOHT) as well as the Clinical Director of the Primary Care Research Network under LKCMedicine. He actively promote primary care research culture, capacity and capability in Singapore. His research interests are on multimorbidity, chronic disease management, digital solutions for primary care and health services research. Jo-Anne Manski-Nankervis,Academic Director, Primary Care and Family Medicine A/Prof Jo-Anne Manski-Nankervis is an academic general practitioner (GP) and Co-Director of Family Medicine and Primary Care Unit and Primary Care Research Network at Lee Kong Chian School of Medicine. Her research focus is on the development and implementation of technology to inform decision making in general-practice, and the use of data to describe and improve general-practice activity, with a focus on chronic disease management.
